I think most of us agree that using a cell phone while driving is distracting.
I've also found myself distracted and/or driving unsafely by these things:
Kids arguing and/or acting up
DH not liking the way I drive
Being tired
Not being able to read the street signs (or even finding them in some intersections)
Garage sale signs with miniscule writing and arrows on them
Eating while driving
Looking for something while driving
Getting misplaced/lost
Detours
Bad/incorrect directions

My new car lets me play my iPOD through the stereo and I've taken to listening to books and lectures while driving which seems to keep me more alert.
But, when my grandkids are in the back seat and watching their movies the sound of them makes me want to fall asleep! So, we take lots of breaks for little walks and I eat corn nuts one-by-one so the loud crunch in my head keeps me awake.

My husband usually drives and I navigate. When I do drive, I don't talk on the phone, but I do have my drink handy. I can't eat and drive due to getting very messy. The satellite radio is programed so all I have to is push a button, or I listen to a CD.
Here is my daily reminder of why I don't use a cell phone while driving.
