Another thing I found - long ago when I first started (and occasionally it still catches me) - is when the thread does not pull properly from the spool (it can get caught on something and not pull), it will leave gaps, or a hoop hitting something behind it (like the wall (grin) or a lampbase, etc.) will pull the design out of alignment. There are so many things to check and most are not really errors but just things that happen.
