    Even if fused, I use heavy stiffener which tears away afterwards. I applique the piece to the top then quilt (or have it quilted) normally. Remember to drive your needle right along the edge of the fused applique (just off the applique into the background with the rest of the stitch on the applique) turning it as necessary to keep it horizontal to your needle. Would pictures help?
