I also cut 2x2 inch squares of each fabric piece (or as many as I can) and sew them in a line right on the page. Sheet then goes in a sheet protector and into the binder. Love looking through my book to see what I have done! When I get them, I will tape the thank you note on to the back of each sheet!
This is exactly what I have. I started it when people kept asking to see pictures of my quilts. But, I wanted to include more information so I designed an information page that I place in my book across from each picture. It has been so helpful when someone wants to make something similar to my things. I can look back and tell them info that I would have forgotten otherwise.

If you right click on your mouse you should be able to print the start up info she posted. Right click the click on print picture or just print.

I keep a digital folder for each quilt containing pictures, and an Excel file with some vital information that includes date finished, size, recipient of quilt, but I don't keep a lot of information other than that.

I know we're "supposed" to, but if I remember to take a picture, I consider it good. LOL

I know I can attach pictures. Will it attach a document as well? I tried it and it said it won't work. But, I can tell you what's on my document.

First, I have several lines to write in the story behind the quilt
Who was the quilt given to?
    What was the occasion?
    Date Started
    Date Finished
Name of block or book/magazine pattern came from
finished size/ sashing width, block sizes, etc.
    fabric manufacturer
    kind of batting used
which sewing machine I used for piecing
which sewing machine I used for quilting or who/how quilting was done
I cut a swatch of main fabrics used and attach it as well.

Okay, do any of you have any suggestions of other things I should add to my document?

I keep a "Quilt Journal". I use a lined notebook (a pretty covered one if I can find one) and record basically everything I can about the quilt. The pattern name, designer, where found (magazine, online, etc.), fabric choices (sometimes I even add little scraps of fabric to the page) and manufacturer, where fabric purchased, price paid. What prompted me to make that quilt. If it was for a gift, who was it made for and what was the occasion. Any problems I encountered while making the quilt. What I liked/disliked about making the quilt. The date I began and the date I completed the quilt. If I had help making it (friend has mid-arm and will often finish my quilts for me....YES, she is a wonderful friend!!!) I give credit. Finally I take pictures (close-up and distance) of the finished quilt.

I am currently on my second journal and I love looking back and seeing how far I've come as a quilter.

One other thing: I keep a list of "I Want to Make This Quilt" in the journal and as I complete one of these, I enter the date next to it.

    Happy Quilting!

I have a photo album of quilts but not a journal. I have never been good at keeping notes so I know I would never keep up with a journal

I scrapbook my creations. I log the fabric line if I know it (I've gotten better about this through the years), the pattern, and I include fabric scraps or an extra block too. There is an app I've been using that's made documenting as I go so much easier-it's called the project life app. Right now it's for apple only, but it's sensational for this type of thing.

Wheni did did the wedding ring quilt last year, I was able to take pictures and write up every step of the process. If I ever make one again, I'll know exactly what I did.

Plus us ones ice gifted I usually stick the thank you note in there too. Fun to look back and reminisce.

I have kept (not faithfully) a log since I started quilting around 1989. Notes on fabrics, patterns, etc. Put a picture of the finished product as I do a lot of table runners and table toppers as well as all sizes of quilts. I use an engineering notebook because of its graph paper so I can draw blocks in my notes if I want. I also make drawings in EQ and paste in the notebook of quilts I would like to make. In EQ I make notes about the quilt to refresh my memory. It is amazing to look back and see what I have done over the years.
