Default EZ Quilting Circle Cut received as gift - NOW WHAT?

I got this as a gift and I am not sure what to do with it. I have read the instructions and it sounds like I have to cut out a circle of fabric to "insert" and then cut out a circle space into a square block. Am I being dense about this?

Thanks Woody for the youtube video! I had the ruler and just gave it away because I was using a regular rotary cutter and I tried both the 28 and 45 size with problems. Now I didn't know there is a rotary made just for an easy cut! I'm going to buy one of those plus I guess I have to get another Circle Cut. I have other types of those rulers too so it looks like the special rotary cutter will work far better then the others.

When you get it and try to cut smaller circles (2-3") and still have trouble, don't get discouraged, just turn the cutter around. I can't remember which right now but the blade will need to either face you or away from you--I'm having a senior moment right now . It works just fine with the larger circles.
