Instructions for quilt bindings are usually for the standard narrow binding (cut 2-1/12 inch strips, fold, press, sew to front w/ 1/4" seam, turn, hand sew, etc.).
What is the best way to do a wide binding? How do I figure out the math....where to place on my quilt and how to miter the corners?

Do I have to use bias binding for rectangular quilts?
My backing fabric is only 45" wide, so how can I best piece it for a large top?
My backing is too small and I don't have any more left. How can make my backing large enough?
Why do I need to cut my backing and batting larger than the top, and how much larger is large enough?
Do I have to prewash my fabric?
