    Our LQS does classes for the kids and they tie all of theirs. They use embroidery floss - all 6 strands. I would do a hand-width apart in each direction - about 4-6 inches per intersection. I can only guess on the surgeons knot vs. the square not.

    When tying a quilt, I prefer using embroidery floss, some people use yarn or crochet thread, personal preference I think.

    this is the way I tie my quilts but I use the quilt frame to keep the layers tight and even. also sitting is easier than bending over or being on my knees. I made my frame the as cheryl rearick's granny and my Mom did.
