    Why don't you just treat the quilt inside the duvet as if it is a new quilt and "tie quilt" it? That should certainly hold everything together. Forget about ties in the corners, pins, etc. Get a big curved needle, some strong yarn or you can use embroidery floss, (or your choice) and just get to work tying! Do a search for 'hand tied quilts' for info.
