Perhaps a photo of her two dogs together would be also, knowing how much they meant to each other too. Someday, both will be gone and then the photo becomes a bittersweet memory of a time in her life and her two best furry friends.
The date you made the quilt (i.e.: 2011) would be all I'd recommend. Plus a title for the quilt, such as one of the poems/sayings mentioned in other responses?

That is so tramatic for this poor girl. I had an Afghan Hound that was bouncing around, playing out in the yard with the other dogs. I went to let Madam in and she rain into the kitchen and just dropped. Please express my sincere sympathy to her.
I think it would be a wonderful memorial to honor her pet by putting the picture on the back of the quilt. What a beautiful idea.

Maby years ago my little poodle died unexpectently. At this time I was working on a counted cross stitch block. I placed her name and date of birth and death on it. That was 27 years ago and she is remembered by the quilt it went into. I think it would be proper to put her fur baby name and dates on it and the picture you have. Also give her a copy of "The Rainbow Bridge"
THE RAINBOW BRIDGE
There is a bridge connecting Heaven and Earth It is called the
Rainbow Bridge because of the many colors.
Just this side of the Rainbow Bridge there is a
Land of meadows, hills and valleys with
Lush green grass.
When a beloved pet dies, the pet goes to this place.
There is always food and water and warm Spring weather.
The old and frail animals are young again.
Those who are maimed are made whole
Again. They play all day with each other.
There is only one thing missing.
They are not with their special person who
Loved them on earth. So each day
They run and play until the day comes when
One suddenly looks up! The nose twitches.
The ears are up. The eyes are starring.
And this one suddenly runs from
The group.
You have been seen, and when you and your
Special friend meet, you take him or her
In you arms and embrace. Your face
Is kissed again and again, and you look once more
Into the eyes of your trusting pet.
Then you cross the
Rainbow Bridge together,
Never again to be separated.

The vet said that it was a heart attack, the dog was only 5 yrs of age.
Anyway, her mom told me that she liked to curl up on the couch with both dogs and snuggle into a blanket which I was told is very old, worn and has rips and tears in it.
I am making this quilt (mom knows) for her and hopefully she will see fit to use it with her remaining dog.
(pictur of the top which iI finished this morningis is below)
My question is this....do you think it would be a good idea or not to include something about her deceased fur baby. She gave me a picture of him and I could transfer it on to fabric and sew it on the back with the name and dates of birth and death?
Thanks for you opinions!

What a terrible thing to happen. The quilt is beautiful and as a devoted puppywog, fur baby, lover I would love to have a picture of my friend on the quilt. I like the suggestion made by knlsmith to print it on fabric and use it with the label. God Bless you for understanding the grief.

How thoughtful of you to do this for her. I think it's a great idea and if it was me I would appreciate the picture...kind of still being able to snuggle with the dog in a way... but maybe you should ask her mom first just to get an idea of how the daughter would react.
