I do it indoors on my dining room table. I have pads on the table, but don't seem to have an overspray problem. I use 505 and the smell does not bother me either.

I spray baste on my tile floor, washing it before and after. I don't have too much of a problem with overspray, just a tad now and then when I make a mistake at the edge. It does not seem to seep thru the fabric. And I spray the fabric not the batting.

Read the instructions. It says "adequate ventilation" on my can. You might want to spray very lightly, then walk away so that you don't inhale. I would be very careful with your condition. I am not sure if a spray mask will help.

If you have breathing problems I would not advise using basting spray.
It's nasty stuff. Try glue basting with Elmer's Washable School Glue.
If you do a search here there's lots of discussion on how it's done.
Try on a small piece and see if you like it before doing a big project.

I also have COPD, I use 505 in dining room in open plan house and back door open, with no problems. Air freshness much worse lol.

For lack of a large space I took mine out doors and taped it to the cement block wall. Worked out very nicely although waiting for a non-windy day was very pesky. No worries of smell or overspray.

When I spray I wear a paper mask - the type the construction and landscapers use. It keeps the spray out of my mouth and nose - and I have a ceiling fan overhead going gently so it is pushing it down with the windows and door open.

If you can spray it outside, all the better. If you are super sensitive to chemicals then I would not use it at all and would go with the Elmer's School glue route. Because after it is all together you still have to handle it to quilt it. It does not bother me and fumes can send me into a migraine, but handling the spray basted quilt does not.

I have breathing problems too. I would use 505 or whatever outside or use Elmers inside.
