    Normally they are caused by not figuring out the size of the proper boarder. Instead alot of people just cut the border/sash the length/width of the quilt and then just sewing it on. You need to take different measurements and the average them and then start pinning it to top from the middle out to end. You may have to ease the fabric but this is what will make it straight.
