In case your quilt is ever found in an antique shop 100 years from now, you'll want the buyer to know as much as possible about you so:
Put the name of the quilt
Who it was made for (if you know)
The date you finished the quilt (not the day you started)
The city and state where you reside -- and
Your full name INCLUDING your maiden name, I've been told that the maiden name gives a better identification than simply your married name.

Also -- it's best to apply the label before quilting as the quilting will help keep the label on the quilt (in case some thief attempts to remove the label).

i also include the name of the quilt- care instructions, any information about designer- if i used someone's pattern.or if you have someone else work on the quilt-like having someone else quilt it- that info should be included- or if it was blocks from a swap- if you didn't want to list each person you could at least state the swap..
some of the large quilt shows recommend you actually write all of your information right on the quilt- at the top under the hanging sleeve- that way if someone swiped it you could easily identify it as yours---surprise to them :)

Oh my goodness. You should put a label on everything you make. I've found old quilts in antique/junk shops, yard sales, etc. and part of the excitement is knowing who made the quilt and when it was made. Even within our own families, it would be a shame if 100 years from now, no one knew who had made the quilt. Your work is beautiful -- be proud of it -- and put the information on the label.
