I don't have an answer, but your quilt is adorable! I just had to say. I made a pillow once where I put a piece of batting under the design I wanted to pouf up, then sewed around it. Then I put the regular batting under all and quilted. It was easy and looked great.

Just my opinion....but i always stitch my appliques down and then stitch around them when quilting. I want them to be well secured to the background....and i can make "design" (using the word loosely here) choices about how i want the quilting to be afterward. Also...i quilt with my walking foot - and it is harder to see using that when trying to keep right on the edge of the applique to sew it down. I have a clear standard foot for that. Your quilt, your choice really. Either way will work.

This may have already been discussed, but this is what I was taught in a class I took on landscape quilting. On the parts we wanted to stand out (or be poofy), we cut the same pattern piece out of the batting (only a smidge smaller) then adhered with spray to back of the fabric piece. Stitched it to the background piece, etc,. Then a piece of batting was used to back the whole thing. I'll bet this is mud clear. IOW, the 'poofy" parts will have two layers of batting behind them.Then the whole quilt is quilted as usual.

They certainly are adorable. Love the rick rack. I always got enough pouf with Hobbs 80/20. You can try doing some trapunto stitching:
