Here are the steps I use to make a hanging sleeve.
First cut a piece of fabric 8 inches x the width of the quilt inside the bindings + 1 inch.
I use muslin if I dont have any of the fabric that the backing is made of

Measure the width of the quilt between the bindings and hem the other end of the sleeve.
Fold the sleeve in half to find the middle.
Do the same with the quilt.
Starting at the middle of both, pin the sleeve just under the binding with the seam facing the quilt

Beginning at one end,
if right handed start at the right end, left handed, the left end..
Start by sewing the lower edge of the end of the sleeve to the quilt using a blind hem stitch. Catch the sleeve with the needle, then pass the needle into the quilt back and batting and come up about 3/8 inch away and through the sleeve
