More than likely the stop motion knob washer is in backwards or 180° from where it should be. When this happens the stop motion knob can't be tightened enough to keep from vibrating loose.
The two tabs in the center should face the stop motion knob or face out. Then the washer can be rotated a half turn to give the stop motion knob more rotation to tighten up.

I agree with Joe. While you are in there, however, clean out the old grease with q-tips or a dental instrument and re-grease with Tri-Flow or Singer lubricant (not oil). I haven't worked on a 15-91 lately, but seem to recall that the gears are behind the stop motion washer; right?

I spent almost all day fixing up an old Singer 15. Then I couldn't find my camera. I am always amazed - I don't think those ever die. I should have pronounced this one dead on arrival due to not moving, rust, missing tiny little screws on the tension, no visible means of power and missing the slide plate and no case or table. I found parts here and there. And most of the rust came off - it now hand cranks. It isn't as pretty as Grant's machines turn out but it works as good I'm sure... brag brag brag.........
