Upper right corner, just below and to the right of the words "Fashion Mate" is the numbers 252. That is your model number.
As for the stains in the case bottom, I've yet to see one without them. I clean them out, wash them with Simple Green and forget it.

Thanks for that picture. I was wrong. Apparently you can have a case that works both ways. I have no idea where you could find the hinges. It looks like they're pretty specific for that style case. Ebay maybe?
I've passed by a couple machines of that style due to not knowing how they would perform. I might have a closer look at the next one I see.

The Singer Fashion Mate 237 has a case like that, and as far as I know, there is no type of screw that couples the base of the case with the machine. That metal part in the bottom center of the case base is meant to slide out when you tip the machine back, so that the whole thing doesn't tip over.
